ACG Hospice is seeking a Hospice and Palliative Care Nurse Practitioner (PRN) to provide compassionate, patient-centered care for seriously ill individuals in the home setting. You will assess, diagnose, manage symptoms, and coordinate with a skilled care team to ensure comfort and dignity.

The role emphasizes collaboration with clinicians, ongoing palliative services, and education for patients and families, with access to leadership support and opportunities for professional growth.

Qualifications

• Heart to serve patients and families and a passion for providing the best possible care.
• Education: Accredited Nursing program (Certificate/ Diploma/ Associate or Baccalaureate) preferred.
• Experience: 2 years in chronic disease management/long-term care; hospice background preferred.
• Required: Reliable transportation and ability to perform physical tasks as needed.

Responsibilities

• Care for seriously ill patients to alleviate burdensome symptoms and honor patient goals.
• Collaborate with a multidisciplinary team to deliver high-quality hospice and palliative care.
• Provide education and support to patients, families, and caregivers within the home setting.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
