Job summary

Agape Care Group seeks a Hospice and Palliative Care Nurse Practitioner (PRN) to serve Tulsa, OK, delivering advanced symptom management and care aligned with patient goals in home and facility settings.

You will work with clinicians to provide education and high-quality hospice/palliative care, with leadership support and ongoing professional development opportunities. Flexible PRN hours available.

Qualifications

• Heart to serve patients and families and a commitment to providing the best possible care.
• 2 years of chronic disease management/long-term care experience in the last 5 years; hospice background preferred.
• Reliable transportation and ability to lift 80–100 lbs as needed.

Responsibilities

• Provide hospice and palliative care to seriously ill patients, alleviating burdensome symptoms.
• Collaborate with a multidisciplinary team to deliver education and high-quality care to patients wherever they call home.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred
• Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs and bear the weight of an average adult effectively
