Job summary

Senior Community Care of Colorado PACE is hiring a Nurse Practitioner to provide primary care to PACE participants at Eckert and Montrose sites. You will perform comprehensive assessments, manage chronic and acute conditions, and collaborate with the interdisciplinary team to develop individualized care plans.

The role requires current RN licensure and NP certification with prescriptive authority within a year, plus experience with frail elders. On-call rotation is required.

Qualifications

  • Graduate of a school of professional nursing required.
  • Licensed as RN in the state of employment.
  • Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing.
  • Prescriptive authority required within one year of hire.
  • Certified as a Geriatric Nurse Practitioner preferred.
  • Minimum of one year experience with frail and elderly population.
  • Two years’ experience as NP in geriatric setting preferred.

Responsibilities

  • Provide primary care to participants under Medical Director supervision.
  • Perform physical assessments and semi-annual reassessments; develop and implement plans of care.
  • Evaluate physical complaints and provide appropriate treatment.
  • Educate participants and caregivers.
  • Work as a member of the Interdisciplinary Team (IDT).
  • Assess, plan, and care for frail elder participants per Senior CommUnity Care standards.
  • Coordinate with the IDT to develop comprehensive care plans.
  • Manage chronic diseases and acute illnesses in PACE setting across clinic and home.

Hourly Range: $126k - $140k annually, based on experience.

Schedule: Monday-Friday 8am-5pm; occasional weekend for on-call hours *NPs are required to participate in the on-call rotation.

This position will work at both sites (Eckert & Montrose), sharing time between each.

What You Bring (Requirements)
  • Graduate of a school of professional nursing required.
  • Licensed as Registered Nurse (R.N.) in the state of employment.
  • Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing.
  • Prescriptive authority required within one year of hire.
  • Certified as a Geriatric Nurse Practitioner (G.N.P.) preferred.
  • Minimum of one year of experience in working with the frail and elderly population required.
  • Two (2) years of experience as a nurse practitioner in a geriatric setting preferred.
  • Must have a valid driver's license, proof of insurance and have means of transportation.
  • Working knowledge of the PACE regulations desired.
What You Bring (Requirements)
  • Under the supervision of the Medical Director provides primary care to participants.
  • Performs physical assessments of new PACE Enrollees, semi-annual reassessments of participants and develops and implements appropriate plans of care to Senior CommUnity Care program participants.
  • Evaluates participant physical complaints and provides appropriate treatment.
  • Provides participants and caregiver teaching and education.
  • Functions as a member of the Interdisciplinary Team (IDT).
  • Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and Senior CommUnity Care standards.
  • Performs in person comprehensive history and physical assessments of new Senior CommUnity Care PACE participants.
  • Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Conducts an in-person re-assessment semi-annually and as needed.
  • Manages, evaluates and treats participants with chronic diseases, maximizing interventions in the PACE clinic or the participant's home prior to initiating a higher level of care.

  • Responsible for the oversight of the care of PACE participants who reside in a Skilled Nursing Facility.
  • Participates in the on-call rotation.
  • Performs other duties as required or requested.
