Senior Community Care of Colorado PACE is hiring a Nurse Practitioner to provide primary care to PACE participants at Eckert and Montrose sites. You will perform comprehensive assessments, manage chronic and acute conditions, and collaborate with the interdisciplinary team to develop individualized care plans.
The role requires current RN licensure and NP certification with prescriptive authority within a year, plus experience with frail elders. On-call rotation is required.

Hourly Range: $126k - $140k annually, based on experience.
Schedule: Monday-Friday 8am-5pm; occasional weekend for on-call hours *NPs are required to participate in the on-call rotation.
This position will work at both sites (Eckert & Montrose), sharing time between each.
