Job summary

Senior Community Care of Colorado PACE in Eckert, CO is seeking a compassionate Certified Nursing Assistant to provide daily living assistance and restorative care to participants in their homes and at the Day Center. The role requires a Colorado CNA license, a High School diploma, and at least one year of experience with elderly or frail individuals.

Travel to participant homes is mandatory with mileage reimbursement provided.

Qualifications

  • HS diploma or equivalent required.
  • Current Colorado CNA license required.
  • Minimum 1 year experience with frail or elderly.
  • Valid driver's license and transportation.

Responsibilities

  • Provide daily living assistance and restorative care to participants.
  • Monitor health status and maintain a safe environment for PACE participants.
  • Work as part of the Interdisciplinary Team.
  • May travel to participant homes for care.
  • Assist with IADLs such as shopping, cleaning, and meals.

Pay Range: $19.50 - $23.50 per hour, based on previous work experience.

Schedules: Full-Time, Monday - Friday, 4 or 5 days per week; 8 or 10-hours shifts; Travel Required: To participant homes. Mileage reimbursement provided!

Must be 21 years of age or older.

What You Bring (Requirements)
  • High School diploma or equivalent required.
  • Current License as Certified Nursing Assistant (C.N.A) in the state of Colorado require.
  • A minimum of one year's experience in working with the frail or elderly population required.
  • Must have a valid driver's license, proof of insurance and have means of transportation.
What You'll Be Responsible For
  • The Certified Nursing Assistant is responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to the preservation of assisting participant's in living independently in the community.
  • Assists in monitoring and reporting the health status and providing a safe environment for the PACE participants of Senior CommUnity Care.
  • Functions as a member of the Interdisciplinary Team.
  • This position may require travel to participant homes.
  • Assists participants with Independent Activities of Daily Living (IADL's) including but not limited to grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care as assigned.
  • Reports changes in participant's condition and needs to the Personal Care Supervisor (or designee).
  • Assists with participant activities in the Day Center, including meal service and clean-up.
