Live Well Homecare seeks a Full-Time Advanced Practice Provider (NP/PA) to deliver high-quality primary and urgent care in a sub-acute, long-term care setting. You will work with center leadership and medical directors to coordinate patient care and uphold resident health and well-being.

Must be a graduate of an accredited NP or PA program, licensed and certified accordingly, with preferred geriatric experience. State licensure and NCCPA certification are required.
