Job summary

Liberty Health in Greenville, NC seeks a Hospice & Palliative Care Nurse Practitioner to join our team part-time (2-3 days/week).

You will perform medical visits, coordinate with the Palliative Care team, and provide compassionate care to patients and families under the Liberty Hospice program.

Requirements include a Master’s in Nursing, current NC NP licensure, certification as Adult or Family NP, and hospice experience preferred.

Qualifications

• Master’s degree in Nursing and current licensure in the state of practice.
• Certification as an Adult or Family Nurse Practitioner.
• Hospice or Palliative Care nursing experience preferred.
• Excellent communication and organizational skills.

Responsibilities

• Implements palliative care program requirements.

• Conducts face-to-face encounters for re-certification information to Medical Director/Associate Medical Director for Hospice Patients.

Job Description:

• Implements palliative care program requirements

Job Requirements:

• Excellent communication and organization skills.
