Job summary

Humana Conviva Senior Primary Care in Boerne, TX seeks Nurse Practitioner (NP) or Physician Assistant (PA) to join a multidisciplinary team delivering personalized, high-quality care to seniors in a value-based model.

Onsite at the Boerne clinic, you will report to the Associate Medical Director and participate in on-call rotations while supporting referrals, coordination with hospital/SNF, DME, and home health services.

Qualifications

• Master’s Degree in Nursing or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• TB screening may be required.

Responsibilities

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with the care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately and efficiently with support from quality-based coders.

In this role, you will work onsite at our Boerne clinic located in the San Antonio area. You will report to our Associate Medical Director.

Your Role & Responsibilities

What We're Looking For

Required:

• You will have a Master's Degree in Nursing or completion of a PA program with board certification.
• You will have an active, unrestricted NP or PA license in the state of practice.
• If selected for this role, you will be required to be screened for TB

Preferred:

• You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings.
• You will have an active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$123,800 - $170,300 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
