Job summary

Agape Care Group is seeking a Hospice Nurse Practitioner to provide diagnosis, treatment and comfort care under physician supervision. You will focus on palliative care while supporting families with compassionate guidance and care.

The role emphasizes a patient-centered approach and collaboration with a supportive leadership team. Qualified candidates hold an NP with current licensure and prescriptive authority, plus 2 years in chronic disease management or long‑term care.

Qualifications

  • Education: Accredited master's, post master's, or doctoral nursing program with NP prep.
  • Licensure: Unencumbered APRN license with NP designation and prescriptive authority with DEA.
  • Experience: 2 years in chronic disease management/long-term care; hospice/palliative preferred.

Responsibilities

  • Provide diagnosis, treatment, and care under physician supervision.
  • Focus on palliative care and comfort for patients and families.
  • Support families through health care journeys with compassion and advocacy.

Qualifications
  • A heart to serve patients and families and a passion for providing the best possible care
  • Education: Graduate of an accredited masters, post masters, or doctoral nursing program with formal preparation as a Nurse Practitioner.
  • Licensure: Current unencumbered licensure as an Advanced Practice Registered Nurse with designation as a Nurse Practitioner. Current prescriptive authority with full DEA privileges. Must achieve and maintain national certification in the Nurse Practitioner specialty
  • Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice and/or Palliative background preferred.
  • Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 80-100 lbs.
