Job summary

Hospice of North Alabama is seeking a Nurse Practitioner to provide comprehensive hospice medical care, including evaluation, diagnosis and treatment planning for terminal illness, under physician supervision. You will ensure accurate documentation, collaborate with the interdisciplinary team, and support patients and families in line with state regulations.

The role requires an NP credential, current licensure, DEA as applicable, CPR, and hospice experience.

Qualifications

  • Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
  • Current DEA license, if applicable to state regulations.
  • Current nursing license in state of practice.
  • Current CPR certification.
  • Master's degree with minimum of one year NP experience.
  • One year hospice or palliative care experience preferred.

Responsibilities

  • Providing medical care, palliate and manage terminal illness under supervision of a collaborating physician.
  • Completing history and physical examinations by evaluating the patient's medical condition and health history.
  • Completing hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess eligibility.
  • Making recommendations to the hospice medical director regarding eligibility for certification and recertification.
  • Completing and maintaining accurate clinical notes.
  • Communicating with the physician and participating in interdisciplinary group meetings to meet medical and emotional needs of patient and family.

Responsibilities
  • Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
  • Completing history and physical examinations by evaluating the patient's medical condition and health history
  • Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
  • Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
  • Completing and maintaining accurate clinical notes
  • Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
  • Ensuring compliance with all state and federal legal and regulatory requirements
  • Additional responsibilities as assigned
Qualifications

  • Current DEA license, if applicable to state regulations
  • Current nursing license in state of practice
  • Current CPR certification
  • Master's degree with minimum of one year NP experience
  • One year hospice or palliative care experience preferredAccreditation from an approved certifying body for advanced practice nursing as required by state of practice
  • Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)
  • Commitment to clinical and documentation excellence
