Job summary

Hospice of North Alabama is seeking a Nurse Practitioner to join our team across multiple Alabama locations, including Scottsboro, Gadsden, Oneonta, Pell City, and Guntersville. You will provide hospice care by evaluating patients, diagnosing conditions, developing plans of care, and prescribing medications under collaborating physician supervision.

Qualifications

  • Graduate from an accredited School of Nursing and accredited NP program.
  • Current DEA license, if applicable to state regulations.
  • Current nursing license in state of practice.
  • Current CPR certification.
  • Master's degree with minimum of one year NP experience.
  • One year hospice or palliative care experience preferred.
  • Accreditation from an approved certifying body for advanced practice nursing as required by state of practice.
  • Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing or Geriatrics).
  • Commitment to clinical and documentation excellence.

Responsibilities

  • Providing medical care and managing terminal illness under physician supervision.
  • Completing history and physical examinations.
  • Hospice face-to-face visit requirements to assess eligibility.
  • Recommendations to hospice medical director regarding eligibility.
  • Completing and maintaining accurate clinical notes.
  • Communicating with physician and participating in interdisciplinary meetings.
  • Ensuring compliance with state and federal regulations.
  • Additional responsibilities as assigned.

Responsibilities
  • Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
  • Completing history and physical examinations by evaluating the patient's medical condition and health history
  • Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
  • Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
  • Completing and maintaining accurate clinical notes
  • Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
  • Ensuring compliance with all state and federal legal and regulatory requirements
  • Additional responsibilities as assigned
