Job summary

Hospice of North Alabama seeks a Nurse Practitioner to provide high-quality hospice care. The NP will evaluate patients, diagnose conditions, develop plans of care, and prescribe appropriate medications under physician supervision.

Requirements include a Master's degree in Nursing, NP certification, a current state nursing license, and CPR. At least one year of NP experience is preferred, with hospice or palliative care experience seen as a plus.

Qualifications

• Graduate from an accredited School of Nursing and accredited NP program.
• Current DEA license, if applicable to state regulations.
• Current nursing license in state of practice.
• Current CPR certification.
• Master's degree with minimum of one year NP experience.
• One year hospice or palliative care experience preferred.
• Accreditation from an approved certifying body for advanced practice nursing as required by state of practice.
• Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics).
• Commitment to clinical and documentation excellence.

Responsibilities

• Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
• Completing history and physical examinations by evaluating the patient's medical condition and health history.
• Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
• Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
• Completing and maintaining accurate clinical notes.
• Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers.
• Ensuring compliance with all state and federal legal and regulatory requirements.
• Additional responsibilities as assigned.
