Hospice of North Alabama seeks a Nurse Practitioner to deliver hospice care across multiple Alabama locations. You will evaluate patients, diagnose conditions, develop plans of care, prescribe needed medications, and ensure regulatory compliance.

Qualifications

• Must be a graduate of an accredited NP program and nursing school.
• Current DEA license where required by state regulations.
• Current nursing license in state of practice and CPR certification.
• Master’s degree with at least 1 year NP experience; hospice/palliative care preferred.
• Accreditation from a certifying body for advanced practice nursing as required by state.
• CHPN or geriatrics certification preferred; strong clinical and documentation skills.

Responsibilities

• Provide medical care and manage terminal illness under physician supervision.
• Complete history, physical exams, and develop plans of care.
• Assess hospice eligibility and coordinate with the medical director.
• Maintain accurate clinical notes and documentation.
• Communicate with physician and participate in interdisciplinary meetings.

Responsibilities

• Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
• Completing history and physical examinations by evaluating the patient's medical condition and health history
• Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
• Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
• Completing and maintaining accurate clinical notes
• Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
• Ensuring compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned

Qualifications

• Graduate from an accredited School of Nursing and accredited Nurse Practitioner program
• Current DEA license, if applicable to state regulations
• Current nursing license in state of practice
• Current CPR certification
• Master's degree with minimum of one year NP experience
• One year hospice or palliative care experience preferred
• Accreditation from an approved certifying body for advanced practice nursing as required by state of practice
• Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics)
• Commitment to clinical and documentation excellence
