Job summary

Adoration Health is seeking Hospice Nurse Practitioners to provide comprehensive hospice care, including evaluation, diagnoses, plans of care, and prescribing medications and treatments.

Join a dedicated team, work under a collaborating physician, and ensure high-quality documentation and regulatory adherence to support patients and families in palliative care.

Qualifications

  • Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
  • Current DEA license, if applicable to state regulations.
  • Current nursing license in state of practice.
  • Current CPR certification.
  • Master’s degree with minimum of one year NP experience.
  • One year hospice or palliative care experience preferred.
  • Accreditation from an approved certifying body for advanced practice nursing as required by state of practice.
  • Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics).
  • Commitment to clinical and documentation excellence.

Responsibilities

  • Provide medical care and manage terminal illness in line with state NP practice acts, under physician supervision.
  • Perform history and physical exams, evaluating health status and medical history.
  • Complete hospice face-to-face visit requirements to assess eligibility.
  • Make recommendations to hospice medical director regarding eligibility and recertification.
  • Complete and maintain accurate clinical notes.
  • Communicate with the physician and participate in interdisciplinary group meetings to meet medical and emotional needs.
  • Ensure compliance with all state and federal legal and regulatory requirements.
  • Additional responsibilities as assigned.

Responsibilities
  • Providing medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician
  • Completing history and physical examinations by evaluating the patient's medical condition and health history
  • Completing the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based
  • Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care
  • Completing and maintaining accurate clinical notes
  • Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers
  • Ensuring compliance with all state and federal legal and regulatory requirements
