Creekside Health and Rehabilitation Center is seeking a Licensed Practical Nurse (LPN) to join our care team in Indiana. You will provide daily health care, safety and comfort for residents, with professional assessments and documentation of health status.
You will supervise care assignments, help develop and update care plans, monitor vital signs, medications, and coordinate with staff. A valid LPN license in good standing and basic computer skills are required.
