Job summary

Capital Medical Group, part of Independence Physician Management under UHS, seeks an experienced Nurse Navigator to coordinate thoracic surgery care and drive LDCT lung cancer screening across the DC area. This role focuses on referrals, patient education, SDM counseling, and collaborative care with the Thoracic Surgery division.

Qualifications

• Bachelor's degree in nursing.
• Current D.C. Licensure as an RN or evidence of licensure application in D.C.
• Minimum 2 years of RN experience.
• Experience in public health or health education preferred.

Responsibilities

• Lead the GW Medicine Lung Cancer Screening Program and manage records.
• Coordinate prescreening and screening for LDCT; handle referrals via EMR and providers.
• Provide SDM counseling and arrange smoking cessation when appropriate.
• Place LDCT orders, schedule exams, and ensure results reach providers and patients.
• Support community outreach and assist the Clinical Nurse Navigator as needed.
