Job summary

Qualifications

3 years’ experience on current unit or in specialty area.
BSN or equivalent years' experience.
CPR/BLS certification required.

Responsibilities

Provides specific clinical support to patients and caregivers through the course of care.
Improves access to healthcare for patients and families by addressing Social Determinants of Health.
Assists with transitions of care between outpatient and inpatient settings.
Develops collaborative care plans based on provider treatment plans and patient goals.
Provides education and self-management support tailored to language and literacy.
Identifies cost-effective problem solving and process improvements.
Completes assigned goals.
