Job summary

Baptist Health Memphis invites an experienced RN to join our ambulatory care team. This position is located on the Memphis campus and supports care coordination for the Malignant Hematology and Transplant Clinic, performing all duties as assigned.

The role provides clinical support to patients and families, helps overcome barriers to care, manages transitions between outpatient and inpatient settings, and assists in developing collaborative, patient‑centered care plans.

Qualifications

• 3 years’ experience on current unit or in Specialty area.
• CPR certification required.
• Experience with utilization review processes preferred.

Responsibilities

• Provides clinical support to patients and caregivers through the course of care.
• Improves access to healthcare by addressing Social Determinants of Health.
• Assists with transitions of care between outpatient and inpatient settings.
• Develops collaborative care plans based on provider goals and patient/family objectives.
• Educates patients and families, supporting self-management across language and literacy levels.
• Identifies cost-effective problem solving and process improvements.
• Completes assigned goals.
