WakeMed is seeking an experienced Ambulatory Care Nurse (RN) to join our WakeMed Physician Practices in Cary, NC. The role focuses on coordinating and delivering high-quality, patient-centered care across the outpatient continuum, in collaboration with providers and the care team.

The ideal candidate will have NC RN licensure, prior outpatient experience, and strong skills in assessment, education, and care coordination, supporting WakeMed's commitment to excellence and patient-family-centered

Qualifications

• Current RN licensure in North Carolina.
• Experience as a nurse in an outpatient setting.
• Strong clinical assessment, patient education, and care coordination skills.
• Ability to manage multiple priorities in a fast-paced ambulatory environment while providing exceptional patient service.
• Commitment to WakeMed's patient-centered care model and professional nursing standards.

Responsibilities

• Provide nursing care management and clinical support for ambulatory patients.
• Triage concerns, assess symptoms, and give evidence-based guidance via phone, portal, and in-person.
• Collaborate with physicians and care team to coordinate individualized care plans.
• Identify high-risk patients and coordinate follow-up, referrals, and resources.
• Educate patients and families about conditions, treatment plans, medications, and preventive care.
• Serve as liaison between patients, families, providers, and healthcare team.
• Promote continuity of care and optimal health outcomes in outpatient settings.
• Maintain accurate documentation and support quality and patient experience initiatives.
