Job summary

Qualifications

• Bachelor's Degree in Nursing required.
• RN license issued by the state of practice.
• Typically requires 5 years of nursing experience including assessing and educating patients.

Responsibilities

• Provides continuity of care by ensuring smooth transitions between care settings and developing a relationship with the patient and multidisciplinary team.
• Completes a nursing assessment to identify the patient's individualized needs and collaborates with the multidisciplinary team to develop an interdisciplinary plan of care.
• Supports the patient and family by providing education, addressing psychosocial needs, advocating, and identifying barriers to care throughout diagnosis, treatment and follow-up.
• Provides expert clinical advice to the multidisciplinary team in planning/providing patient/family education and discharge planning including wellness and health maintenance within designated specialty.
• Coordinates timely, seamless, evidence-based care across the care continuum to achieve optimal patient outcomes.
• Evaluates, selects or designs patient education materials on disease site programs and provides education/outreach to patients, Advocate staff and community.
