Geisinger is seeking a Licensed Practical Nurse Navigator to guide defined patient populations through complex care pathways, focusing on access, referral accuracy, sequencing of care, and continuity across services.

You will coordinate referrals, schedule patients, and support RN clinical operations, leveraging clinical knowledge to facilitate introductions to the program and ensure timely follow-up and surveillance.

Qualifications

• Graduate from Specialty Training Program-Nursing (Required).
• Minimum of 2 years nursing experience (Required).
• Licensed Practical Nurse (LPN) license required.

Responsibilities

• Navigate defined patient populations through complex care pathways and coordinate access to services.
• Improve patient access, care coordination, and referrals through timely navigation activities.
• Coordinate referrals and scheduling, and support RN clinical operations.
