Minimally Invasive Procedure Specialists in Highlands Ranch, CO is seeking a Clinical Nurse Navigator, on-site, full-time to coordinate and deliver compassionate care from 8 am to 5 pm, Monday through Friday. You will support patients and providers, review clinical information, prepare charts for consultations and procedures, and guide patients through next steps with clinical readiness.

Active RN with BSN is required; experience with complex chronic disease is highly preferred.

Qualifications

• Active Registered Nurse credential.
• BSN Degree.
• Experience caring for patients with complex chronic disease is highly preferred.

Responsibilities

• Assess patient needs and review clinical information.
• Prepare charts for consultations and procedures.
• Collaborate with providers and care teams to promote clinical readiness.
• Guide patients through next steps and answer questions so they feel supported.
