Job summary

St. Anthony Hospital in Lakewood, CO is seeking an RN Navigator to coordinate care across the continuum for identified patient populations. You will lead a multidisciplinary team to eliminate barriers to care and optimize outcomes for patients and their families.

You must be a Registered Nurse with strong assessment, care coordination, and collaboration skills, including patient navigation and program referrals, to ensure effective communication with physicians and care teams.

Qualifications

• Bachelor's degree in Nursing required.
• Three years of experience, with one year in a specialty.
• Strong psychosocial assessment and clinical skills.
• RN license active; current license in participating state (NLC).
• BLS certification from the American Heart Association.

Responsibilities

• Provide nursing care and guidance to the patient from first contact through the continuum.
• Guide patients on treatment plans and next steps in appointments.
• Assist with scheduling follow-up appointments and provide reminders.
• Educate patient/family on care guidelines and treatment options for informed decisions.
• Educate and facilitate introduction to program clinical trials and referrals.
• Advocate for patients and families to prevent or resolve clinical issues.
• Develop relationships with physicians and staff to ensure effective communication.
• Serve as a liaison between specialists, primary care physicians and patients.
