The Nurse Navigator educates, advocates, and communicates with patients, families, and providers, ensuring a smooth clinical experience and safe, efficient workflows.

Qualifications

• Graduate of an accredited School of Nursing with current RN licensure by the Kentucky State Board of Nursing.
• Bachelor of Science in Nursing required, or higher.

Responsibilities

• Serve as the primary contact for physicians, patients, families and care teams to coordinate services.
• Educate, advocate and communicate to streamline the patient’s continuum of care.
• Develop processes to improve the clinical experience and patient safety in the care flow.
