Qualifications

• Active Registered Nurse license required.

• 3+ years of clinical hospital/managed care experience.

Responsibilities

• Coordinate care across the continuum and liaise between patients, physicians and care teams.
• Educate patients and families; provide information and support to facilitate decision making.
• Monitor patient outcomes and participate in quality improvement activities; ensure patient satisfaction.
