Job summary

Baptist Health Medical Group seeks a Nurse Navigator for Memory Disorders Neurology in LaGrange, KY. This role coordinates care across multidisciplinary teams, educates patients and families, and advocates for streamlined, safe clinical experiences.

The RN must be licensed in Kentucky and hold a BSN or higher, contributing to high-quality patient care. The position involves collaboration with Baptist Health Hardin and additional entities, with a focus on comprehensive care planning and caregiver

Qualifications

• Graduate of an accredited nursing program with current RN licensure by the Kentucky State Board of Nursing.
• Bachelor of Science in Nursing is required, or higher.

Responsibilities

• Serve as the primary contact for physicians, patients, caregivers, and care teams.
• Coordinate comprehensive services across the continuum of care for memory disorders patients.
• Act as educator, advocate and communicator to patients and families.
• Develop processes to streamline the clinical experience and ensure patient safety.
