Appalachian Regional Healthcare (ARH) is seeking a Nursing leader to oversee the GUIDE neuroscience program and care navigation initiatives in the Lexington area. The role emphasizes collaboration with ARH providers and community resources to enroll patients in GUIDE model services.

The candidate should have a Nursing degree with KY/WV licensure or a compact state, plus 2–3 years of related experience and strong program-management capabilities to lead multiple projects within a dynamic

Qualifications

Bachelor’s Degree in Nursing and current licensure in KY/WV or compact state preferred, per the posting.
2-3 years of experience in aging, Alzheimer’s/dementia, patient care navigation or related field preferred.
Leadership or program management experience preferred.
Excellent written and verbal communication skills are required.
Administrative or managerial experience with ability to oversee projects and multiple priorities.

Responsibilities

Promote the mission, vision and value of ARH and the neuroscience service line.
Maintains quality oversight of program and patient care delivery.
Interprets business data to contribute to decision making.
Oversee daily operations of CMS GUIDE model initiatives and patient care navigation.
Collaborate with ARH providers and community resources to enroll patients in GUIDE model services.
Coordinate care with primary care provider and other specialists caring for the patient.
Development of comprehensive care plans with the interdisciplinary care team.
Education of internal and external partners on available services.
