Munson Healthcare in Traverse City, MI, invites applications for a Nurse Navigator - Clinically Integrated Network. This full-time role supports care management and coordination across adult and pediatric patients with complex needs.

The Nurse Navigator partners with clinicians to optimize health outcomes, reduce unnecessary hospital use, and promote patient-centered care in a community hospital system.

Qualifications

• Currently licensed as a Registered Nurse in the State of Michigan.
• Bachelor of Science degree in nursing preferred.
• Minimum 5 years experience as an RN is highly preferred.

Responsabilités

• The Nurse Navigator acts as a liaison between patients, staff and physicians to provide care management and coordination.
• Collaborates with primary care providers, specialists, and families to ensure quality, efficient services.
• Assesses plans, coordinates, monitors and evaluates options to optimize patient health and reduce hospital use.
• Develops patient-centered care plans incorporating evidence-based guidelines.

A Day in the Life

The navigator functions in an organized and time conscious manner using effective customer service and interpersonal skills at all times.

• The Nurse Navigator will act as a liaison between patients, professional staff and physicians by providing care management and care coordination for adult and pediatric patients, with complex conditions, with complex social needs, and education needs.
• Serves in an expanded health care role to collaborate with primary care providers, specialists, members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services.
• Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient’s health status and decreasing hospital and emergency room utilization.
• Integrates evidence-based clinical guidelines and protocols, in the development of individualized care plans that are patient-centric, promoting quality and efficiency in the delivery of health care

What’s Required

• Currently licensed as a Registered Nurse in the State of Michigan.
• Bachelor of Science degree in nursing preferred.
• Minimum 5 years experience as an RN is highly preferred.

Munson Healthcare requires all employees be vaccinated or have lab confirmed immunity for Measles, Mumps, Rubella and Varicella. MHC also requires all employees to receive a flu vaccine during the flu season in the year that they are hired and annually thereafter, or receive an approved medical or religious exemption.
