The University of Miami seeks a Nurse Navigator 1 to be the first point of contact for patients entering the health system, ensuring timely scheduling of the initial appointment and coordinating care throughout the continuum.

The role supports patients, families and the multidisciplinary care team; educates on treatment plans; identifies barriers to care and facilitates referrals to appropriate services, collaborating to optimize the patient journey.

Formación

• Bachelor’s degree in a relevant field.
• Florida RN license; at least 2 years of relevant experience.
• Strong communication and data-analysis skills; proficient with Microsoft Office.

Responsabilidades

• Triage new patient appointments and schedule initial visits promptly.
• Review outside records to verify scheduling accuracy.
• Identify barriers to care and refer to support services.
• Educate patients on treatment plans and care continuum.
• Coordinate plan of care with multidisciplinary team.
