The University of Miami/UHealth Department of Obstetrician & Gynecologist (OBGYN) has an exciting opportunity for a full-time Nurse Navigator 1 to work in Miami, FL.

The Nurse Navigator 1 is the initial point of contact for a patient entering the health system and assures timely scheduling of the first appointment, and coordination of care after completion of the first appointment and supports the patient throughout the care continuum.

Qualifications

• Bachelor's degree and at least 2 years of relevant work experience.
• Valid State RN license (Florida).
• Demonstrates learning agility and ability to adapt to changing priorities.
• Proficient in office software and healthcare IT systems.
• Strong communication with patients, families and team.

Responsibilities

• Triages new patient appointments and schedules initial visits.
• Reviews outside medical records for scheduling.
• Assesses barriers to care and refers to support services.
• Educates patients on treatment plans.
• Coordinates care throughout the patient journey.
• Counsels patients on healthy practices.
• Collaborates with multidisciplinary care teams.
• Mentors staff and coordinates preceptors.
• Maintains professional knowledge and participates in continuing education.
