Job summary

The University of Miami Health System is seeking a Nurse Navigator 1 (H) to serve as the first point of contact for patients, coordinating initial appointments and guiding them through the care continuum in a remote work setting.

This role requires a Florida RN license, a bachelor’s degree, and at least two years of relevant experience, with strong interpersonal, communication, and data skills to support patients, families, and the multidisciplinary care team.

Qualifications

• Bachelor’s degree in a relevant field is required.
• Valid State of Florida Registered Nurse (RN) license is required.
• Minimum 2 years of relevant work experience.

Responsibilities

• Triages new patient appointments to appropriate providers and ensures timely scheduling.
• Reviews outside medical records for appropriate scheduling.
• Assesses barriers to care and refers to support services when needed.
• Educates on the treatment plan for patients based on diagnosis.
• Supports patients throughout the care continuum.
• Counsels individuals and patients on positive health practices.
• Collaborates with a multidisciplinary team to outline best treatment.
• Performs holistic evaluation of specialty population using proven techniques.
• Implements improvements in patient care, and healthcare policies and resources.
• Mentors other healthcare professionals as preceptor or coordinator.
• Maintains professional knowledge through organizations, CE programs, conferences.
• Adheres to University policies and safeguards assets.
• Coordinate educational activities and patient education across the care continuum.
• Identify barriers to learning and document patient barriers.
• Educate patients and families about disease processes and treatment options.
• Assist with treatment decision making and post-operative education.
• Provide survivorship education and transition planning.
• Advocate for patients and support end-of-life/palliative decisions.
• Coordinate with staff to ensure timely follow-ups and referrals.
• Develop relationships with community organizations for cancer education and support.
• Collaborate with outreach programs to increase public cancer screening awareness.
• Lead and participate in multidisciplinary tumor board coordination as needed.
