Job summary

Munson Healthcare in Traverse City, MI is seeking a Nurse Navigator to provide care management and coordination for adult and pediatric patients with complex needs. You will liaise between patients, physicians, and staff to ensure high-quality, efficient care.

This full-time role requires strong interpersonal skills and a patient-centered approach. You will collaborate with primary care providers and specialists to develop individualized care plans, integrate guidelines, and optimize health

Qualifications

• Currently licensed as an RN in Michigan.
• BSN degree preferred.
• Minimum 5 years RN experience preferred.

Responsibilities

• Act as liaison between patients, staff, and physicians to provide care management and coordination for adults and children with complex needs.
• Collaborate with primary care providers, specialists, and families to ensure quality, efficient, and cost-effective care.
• Assess, plan, implement, coordinate, monitor, and evaluate options to optimize patient health and reduce hospital and ER use.

Job Description

A Day inthe Life

The navigator functions in an organized and time conscious manner using effective customer service and interpersonal skills at all times.

• The Nurse Navigator will act as a liaison between patients, professional staff and physicians by providing care management and care coordination for adult and pediatric patients, with complex conditions, with complex social needs, and education needs.
• Serves in an expanded health care role to collaborate with primary care providers, specialists, members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services.
• Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient’s health status and decreasing hospital and emergency room utilization.
• Integrates evidence-based clinical guidelines and protocols, in the development of individualized care plans that are patient-centric, promoting quality and efficiency in the delivery of health care

Qualifications

What’s Required

• Currently licensed as a Registered Nurse in the State of Michigan.
• Bachelor of Science degree in nursing preferred.
• Minimum 5 years experience as an RN is highly preferred.

Additional Information
