Job summary

Santa Barbara Cottage Hospital is seeking a skilled Nurse Navigator HME to coordinate durable medical equipment for patients, assess needs, and ensure aligned care plans with physicians, families, and vendors. The role requires an active RN license in good standing, 2–3 years of clinical nursing experience (home health or DME environment preferred), and Graduate of an accredited School of Nursing.

This on-site position reports to the HME Operations leadership.

Qualifications

• RN license active and in good standing.
• Graduate of an accredited School of Nursing.
• Minimum of 2–3 years of clinical nursing experience; home health, utilization review, case management, or DME environment preferred.

Responsibilities

• Clinical Review & Utilization Management: evaluate medical records and prescriptions to determine medical necessity of DME requests.
• Patient & Caregiver Education: instruct on equipment operation, safety, and maintenance via virtual or telephonic guidance.
• Interdisciplinary Collaboration: partner with physicians and home health case managers to modify care plans and overcome equipment access barriers.
• Compliance & Quality Assurance: ensure HIPAA/EEOICPA and company policy adherence.
