Job summary

Munson Healthcare in northern Michigan is seeking a Registered Nurse to join a dedicated care team at our Indian River and Charlevoix locations, with time split between both sites.

This full-time, 40-hour weekly day-shift role features a day shift (8 AM–4 PM, Mon–Fri) and emphasizes care management, coordination with physicians, and patient education to optimize health status and reduce unnecessary hospital use.

Qualifications

• Licensed RN in the State of Michigan.
• Experience in complex chronic disease management preferred.
• Bachelor of Science in Nursing preferred.
• Five years' (5) experience as an RN preferred.

Responsibilities

• Act as a liaison between patients, professional staff and physicians by providing care management and coordination for adult and pediatric patients with complex conditions, social needs and educational needs.
• Serves in an expanded health care role to collaborate with primary care providers, specialists, and patient/families to ensure the delivery of quality cand cost-effective health care services.
• Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient's health status and decreasing hospital and emergency room utilization.
• Support quality improvement initiatives and adherence to safety standards.
