Responsabilités

• Perform clinical assessments and identify patient needs and barriers to care.
• Provide patient and family education to support understanding, adherence, and informed decision-making.
• Coordinate care with multidisciplinary teams, including EMS and hospital partners.
• Ensure accurate documentation in electronic medical records (EMRs).
• Maintain strict safety and quality standards to promote safe transitions of care.
• Deliver compassionate, patient-centered support that improves health outcomes.
