Qualifications

• Graduation from an accredited school of nursing or practical nursing with Alabama standards; 3 years related experience required.
• Master's Degree will substitute for 1 year of the required experience (preferred).
• Current Alabama RN or LPN license, or current Multi-State RN License per NLC Alabama, upon hire.

Responsabilités

• Coordinates care with referring clinicians and referral patients to simplify access and enhance communication and the patient/family experience.
• Assesses educational needs in collaboration with nursing and medical personnel and evaluates educational program effectiveness.
• Accesses multiple clinical information systems to gather and verify patient demographics and clinical information.
• Assists in identifying and documenting issues related to transitions of care and rounds on patients to ensure expectations are met.
• Provides individualized support, education and information to make care seamless and comprehensive.
• Facilitates patient scheduling with schedulers and coordinators for appointments and testing.
• Identifies and designs new strategies to increase service utilization.
• Reviews patient lists to identify patients benefiting from interventions and tracks outcomes of referrals and treatments.
• Completes all mandatory department, educational and hospital requirements.
• Adheres to infection control and safety standards.
• Regular attendance and ability to work defined schedule and overtime.
