Hackensack Meridian Health is seeking a Nurse Navigator who collaborates with surgeons and providers to guide patients through care—from initial assessment to treatment and survivorship planning.

Responsibilities include coordinating outpatient care, educating patients and families, and maintaining resource databases while tracking patient flow and reporting activity monthly to leadership.

Qualifications

• RN licensed with at least 5 years of clinical experience.
• Proficient in MS Office and care coordination tools.
• Strong organizational, time management, and critical thinking RN skills.
• Ability to work independently and exercise sound judgment with physicians, APNs, patients, and families.

Responsabilités

• Coordinate outpatient care and identify new patients requiring navigation services.
• Provide patient/family education and assess ongoing needs throughout treatment.
• Develop disease site expertise and connect patients with community resources.
