Job summary

Hackensack Meridian Health is seeking a Nurse Navigator to coordinate patient care alongside surgeons and the healthcare team. The role includes ongoing assessment of patient and family needs across education, financial, psychosocial, nutritional, and clinical aspects, ensuring seamless access to services.

You will connect patients and families to resources within the health system and community, assess eligibility for clinical trials, and support access to care with sound professional judgment.

Qualifications

• Graduate of licensed school of nursing.
• RN with at least 5 years of clinical experience in a relevant clinical specialty.
• Proficient computer skills- proficiency in MS Office.
• Strong organizational, time management, critical thinking RN skills.
• Ability to work independently and exercise sound judgment in interactions with physicians, APNs, patients, and their families.

Responsibilities

• Navigate patient care in collaboration with surgeons and providers, assessing needs across education, financial, psychosocial, nutritional, and clinical areas.
• Connect patients and families to available resources within the health system and community.
