Job summary

Hackensack Meridian Health is seeking a Nurse Navigator to guide patients through outpatient care, collaborating with surgeons and the care team to assess needs, education, and access to resources.

You will build relationships with physicians, coordinate services, and participate in multidisciplinary conferences, while ensuring quality standards and community education as part of the program.

Qualifications

• RN graduate of licensed school of nursing.
• RN with at least 5 years of clinical experience in a relevant clinical specialty.
• Proficient computer skills - MS Office.
• Strong organizational, time management, critical thinking RN skills.
• Ability to work independently and exercise sound judgment with physicians, APNs, patients, and families.

Responsibilities

• NAVIGATION: Coordinates outpatient care, identifies new patients, assesses needs, educates patients and connects to resources.
• SURVIVORSHIP CARE PLAN: Gathers required information and formulates survivorship plan at end of treatment.
• RELATIONSHIPS: Builds relationships with surgeons and providers to support patient care.
• MULTIDISCIPLINARY CARE CONFERENCES: Prepares and documents disease site conferences for treatment planning.
