Qualifications

Our Orthopedics Practice is seeking a Nurse Navigator to coordinate and oversee patient care before surgery, throughout the postoperative period, and after discharge by collaborating closely with providers, patients, and families to ensure timely, high-quality care.


Overview of responsibilities include but are not limited to:


  • Provide comprehensive patient education regarding procedures, treatment plans, and the continuum of care.

  • Coordinate pre-procedure evaluations, testing, appointments, and clearances to support timely care.

  • Assess patient needs and develop individualized care plans, including identifying potential risks or barriers.

  • Serve as a liaison among patients, families, providers, and multidisciplinary healthcare teams.

  • Monitor compliance with established care processes, protocols, and treatment plans.

  • Identify and communicate issues that may affect the timing or effectiveness of patient care.

  • Develop educational resources and tools to promote patient engagement and self-management.

  • Collect, analyze, and report patient outcomes and quality data to support continuous improvement.

  • Coordinate discharge planning, follow-up care, and post-discharge services to promote safe transitions and reduce readmissions.

  • Monitor patient and family satisfaction, participate in quality improvement initiatives, and support the development of care management policies, procedures, and electronic tools.


Qualifications:

Education: Graduate of an accredited school of nursing, Bachelor’s degree preferred


Licenses/Registrations:

C urrent RN license within the Commonwealth of Massachusetts


Experience:

Three to five years of clinical related job experience, case management experience preferred


Special Aptitudes & Activities:

Strong written and verbal communication and interpersonal skills, with the ability to collaborate effectively with patients, physicians, and office staff. Demonstrates solid knowledge of practice workflow, strong organizational and prioritization abilities, and excellent attention to detail while working independently.
