Job summary

Roof Above in Charlotte, NC seeks a program nurse to address physical and behavioral health needs of clients within a multi-disciplinary team. You will meet clients in the Single Site office, in apartments, or in the community, administer medications, provide health education, and coordinate care ensuring confidentiality and safety.

The role requires an accredited nursing program graduate with NC RN licensure, experience with homelessness and mental health/substance use, and strong communication

Qualifications

  • Must be a graduate from an accredited school of nursing.
  • Current licensure in the State of North Carolina as a Registered Nurse.
  • Experience working with chronically homeless individuals preferred.
  • Experience working with individuals with mental illness and substance use required.
  • Knowledge of, and commitment to, harm reduction and Housing First principles.
  • Strong written and oral communication skills.
  • Computer proficiency and familiarity with electronic documentation.
  • Superb collaborative skills and demonstrated success working in a high-needs environment.
  • Creativity, strong problem-solving skills.

Responsibilities

  • Meets with program clients in the Single Site office, in clients apartments, or in the community.
  • Administer medication.
  • Provides education and counseling to clients regarding identified health needs, health care activities, medication effects/side effects, and contraindications.
  • Evaluates clients response to education, counseling, treatment immunizations, medications, and health services provided.
  • Assists clients with obtaining prescribed medications, as necessary.
  • Documents clinical assessment and care provided in the HMIS.
  • Demonstrates appropriate critical thinking and interpersonal skills.
  • Recognizes and provides for cultural and language differences of the tenant.
  • Participates in multi-disciplinary team conferences, staff meetings, and in-services.
  • Educates other staff members regarding clients diagnoses and treatments.
  • Provides written and/or oral status reports of tenants to assure necessary follow-up actions.
  • Attends and participates in continuing education designed to remain current with changes in the profession and maintain professional licensure.
  • Maintains current CPR and First Aid certification including use of an AED.
  • Ensures that an adequate supply of personal protective equipment is available to personnel who may be exposed to blood or other body fluids.
  • Ensures confidentiality regarding sensitive material including protected health information.
  • Other duties as assigned.

Roof Above is NCs most comprehensive homeless service provider. Our mission is to unite our community to end homelessness, one life at a time. Focusing on individuals experiencing homelessness, Roof Above offers everything from street outreach to shelter to Rapid Rehousing to Permanent Supportive Housing. Roof Aboves services and programs reach over 1,200 individuals every day across seven campuses in Charlotte (up to 14 campuses in COVID). Roof Above was born through the merger of Urban Ministry Center and Mens Shelter of Charlotte in 2019.

POSITION SUMMARY:

The program nurse works as part of a multi-disciplinary team to address physical and behavioral health needs for clients .

ESSENTIAL FUNCTIONS:
  • Meets with program clients in the Single Site office, in clients apartments, or in the community.
  • Administer medication.

  • Ensures that an adequate supply of personal protective equipment is available to personnel who may be exposed to blood or other body fluids.
  • Ensures confidentiality regarding sensitive material including protected health information.

Qualifications
EDUCATION, EXPERIENCE AND SKILLS REQUIRED:
  • Must be a graduate from an accredited school of nursing.
  • Current licensure in the State of North Carolina as a Registered Nurse.
  • Experience working with chronically homeless individuals strongly preferred.
  • Experience working with individuals with a history of mental illness and substance use required.
  • Knowledge of, and commitment to, harm reduction and Housing First principles.
  • Strong written and oral communication skills.
  • Computer proficiency and familiarity with electronic documentation.
  • Superb collaborative skills and demonstrated success working in a high-needs environment.
  • Creativity, strong problem-solving skills.

OTHER REQUIREMENTS:
  • Must pass pre-employment and/or random drug and alcohol screenings as a condition of hire and/or ongoing employment.
  • Must have reliable transportation to work.
  • Must be able to lift 25-30 lbs.
  • Must possess a valid drivers license and safe driving record.
WORKING ENVIRONMENT:

Individuals must be able to work with limited supervision, embrace a fast-paced environment, and be comfortable de-escalating negative behaviors with diverse individuals including persons with mental illness and substance use issues.
