Pruitt Health in Pensacola, FL seeks a Clinical Nurse Liaison to coordinate referrals from hospitals and community providers. You will assess patients for admission to home health or hospice and bring clinical expertise to our teams.

This role blends clinical knowledge with sales acumen, developing partnerships with facilities, discharge planners, and payers to secure timely referrals.

Requires two years of college and an RN license; a bachelor’s degree is preferred.

Responsabilidades

• Serve as facilitator for hospital and community referrals to Pruitt Health.
• Assess patients for admission to home health or hospice and advise teams.
• Develop regional referral relationships with facilities and payers.
• Complete discharge calls, documentation, and communication of referral needs.
• Represent Pruitt Health at community events and seminars.
• Plan and execute quarterly/annual territory objectives with staff input.
• Maintain strong liaison with Home Health/Hospice staff to ensure seamless care.
• Prepare and deliver sales presentations to referral sources.
