Job summary

Victory Housing, Inc. seeks a dedicated Health, Wellness, and Clinical Program leader to implement, coordinate, and supervise a comprehensive program for all Assisted Living residents in line with internal policies, COMAR, and nursing standards.

The role emphasizes clinical leadership, vendor coordination, and continuous quality improvement. The position includes ensuring proper documentation, care planning, and oversight of medication management while guiding a multi-disciplinary team to uphold

Qualifications

Must be a graduate of an accredited school of nursing with Maryland RN license.
Three years experience in nursing, preferably in geriatric services.
Baccalaureate degree in nursing or related health field preferred.
Must have successfully completed the 20 Hour Delegating Nurse Course within 60 days of employ.

Responsibilities

Provide nursing assessments, care planning, care oversight and quality assurance for all residents.
Maintain charts including progress notes, nursing interventions, treatments and assessments.
Coordinate third-party vendors (podiatrists, dentists, labs, pharmacy, home health, hospice).
Act as Clinical Leader, advocate for residents, and delegate/ supervise staff within regulatory standards.
Schedule and conduct resident wellness check-ups and ensure proactive clinical care.
