Job summary

Qualifications

• Graduate in nursing with eligibility for RN licensure.

Responsibilities

• Provide and supervise direct patient care in the Community Living Center.
• Administer medications and monitor treatment plans per guidelines.
• Lead care coordination and patient education to improve outcomes.
• Collaborate with interdisciplinary team to support aging and complex needs.
• Ensure accurate documentation and quality improvement participation.
