Responsibilities

• Supports patients with ADLs and personal care
• Assists mobility and transfers under RN direction
• Records vitals and intake/output; reports changes to RN
• Aids nutrition and hydration per diet orders
• Maintains clean, safe patient environment
• Communicates changes in patient condition to RN promptly
• Supports education and orientation for patients and families
• Performs clerical tasks and EMR data entry
• Manages supplies and equipment on the unit
• Performs competency-validated technical skills under RN supervision
