Summary of Job Duties: The Nurse Educator will work within the Family Medicine Clinic to implement and promote an Evidence-Based program focused on Patient and Family Centered Care. Responsibilities include developing clinical strategies for Well Child Checks (0-5 years), coordinating care transitions, and overseeing Community Health Worker call encounters. The role involves collaboration with nursing staff and participation in residency and nursing education activities. Located in Texarkana, AR.

Qualifications:

Minimum Qualifications:

  • Graduate of an accredited Nursing Program with an associate degree and 7 years of experience as an educator/instructor or RN, or BSN in Nursing plus 5 years of experience.
  • One year of clinical experience.
  • Valid Arkansas RN license.
  • Current BLS certification or must obtain within 30 days.
  • Valid AR driver's license, insurance, reliable vehicle, and occasional out-of-state travel.
  • Personal cell phone required.
  • Ability to work evenings and weekends as needed.

  • Proficiency in program management software and EHR systems.

Promotes Screenings, Early Identification, and Clinical Navigation for Children Prenatal-5:

  • Become a high-quality ECD specialist to ensure pediatric/prenatal practices are followed.
  • Use clinical expertise to guide care management.
  • Supervise a Community Health Worker providing navigation, home visits, and education.
  • Work with the clinic team to ensure project goals and health equity.
  • Coordinate referrals and support services for pediatric patients.
  • Document services, billing, and paperwork accurately and timely.
  • Support quality improvement initiatives.
  • Provide training and education on early childhood development.
  • Participate in professional development and meetings.
  • Maintain competency with technical skills and documentation.
  • Maintain professionalism and positive relationships.

Develops and manages relationships with program participants and community stakeholders.

  • Coordinate communication with stakeholders.
  • Support clinic’s Family Voices participation.
  • Build and manage external partnerships.
  • Track and report program deliverables.
  • Identify and address barriers to meet program objectives.

Inform programs and support culturally adapted services.

May perform other duties as assigned.
