Summary of Job Duties: The Nurse Educator will work within the Family Medicine Clinic to implement and promote an Evidence-Based program focusing on Patient and Family Centered Care. Responsibilities include developing clinical strategies for Well Child Checks (0-5 years), coordinating pediatric care transitions, overseeing Community Health Worker calls, collaborating with nursing staff, supporting residency training, and providing pediatric/prenatal education. The role involves working in Texarkana, AR.

Qualifications:

Minimum Qualifications:

• Graduate of an accredited Nursing Program with an associate degree and 7 years of educator/RN experience OR BSN in Nursing with 5 years of experience.
• One year of clinical experience.
• Valid Arkansas RN license.
• Current BLS certification or must obtain within 30 days.
• Valid AR driver’s license, automobile insurance, reliable vehicle, occasional out-of-state travel.
• Personal cell phone required.
• Ability to work evenings and weekends as needed.

• Become an ECD specialist to ensure pediatric/prenatal E-B practices.
• Use clinical expertise for wellness stages and health risk stratification.
• Supervise a Community Health Worker providing navigation and education.
• Collaborate with clinic team for health equity and wellness.
• Ensure screenings and assessments are age-appropriate.
• Coordinate referrals and support services.
• Document services and billing accurately.
• Support quality improvement and professional development.
• Participate in meetings and trainings.

Building Relationships with Program Participants and Community Stakeholders:

• Coordinate communication and support community involvement.
• Foster participation in Family Voices and other advisory groups.
• Manage relationships with external partners.
• Track and report program deliverables.
• Address barriers with problem-solving skills.
