Qualifications

• Experience in adult education and clinical training preferred.

Responsibilities

• Evaluate and educate patients, caregivers, and facility staff on services and therapies.
• Coordinate patient transitions from hospital to home care settings.
• Deliver in-services and clinical training sessions for internal staff.
