Summary
The Community Care (CC) Coordinator Registered Nurse (RN) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care. This nurse has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice.
Qualifications
Basic Requirements:
- English Language Proficiency
- In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless proficient in basic written and spoken English.
- Graduate of an accredited school of professional nursing (ACEN or CCNE) at the time of program completion.
- OR individuals in a master's level bridge program with coursework equivalent to a bachelor's degree in Nursing, with opportunity for licensure prior to program completion.
- Upon licensure, appointment can be temporary with potential for permanent status upon graduation.
- Foreign nursing graduates with current, unrestricted registration meet graduation requirements; foreign education higher than an associate degree requires degree validation.
- Current, full, active, and unrestricted registration as a professional nurse in the US.
- Graduate Nurse Technician (GNT) Exception: Candidates without licensure may be appointed temporarily while pursuing licensure, with a 120-day appointment extendable up to two years.
- Preferred Experience: Care coordination experience preferred.

- Nurse I - Level I: Delivers fundamental care; requires an ADN or diploma.
- Nurse I - Level II: Demonstrates integration of biopsychosocial concepts; requires an ADN or diploma plus 1 year of specialized experience or a BSN.
- Nurse I - Level III: Demonstrates proficiency and self-direction; requires an ADN or diploma plus 2 years of experience or a BSN plus 1 year.
- Nurse II: Demonstrates leadership; requires a BSN plus 2 years of experience or an MSN with one year of experience.
- Nurse III: Demonstrates leadership and complex care management; requires an MSN or a related doctoral degree plus experience.
Physical Requirements: Understanding of the needs of middle-aged and geriatric patients; sensitivity to age, developmental, and cultural factors.
Duties

- Assesses, plans, implements, and evaluates Veteran care.
- Supports high-quality, timely community-based care.
- Coordinates care focusing on patient education, self-management, and satisfaction.
- Follows established procedures and policies.
- Collaborates with interdisciplinary team members to influence care outcomes.
