Job summary

• Coordinates care focused on patient education, self-management, and satisfaction throughout the continuum of care.
• Influences care outcomes by collaborating with the interdisciplinary team.
• Executes leadership and management duties to improve complex resident care.
• Supports quality improvement activities based on data from the CLC.
