Job summary

ERP International, LLC is seeking a full-time Registered Nurse - Case Management to support wounded and injured Active Duty members at Luke AFB, AZ. The role focuses on care coordination, discharge planning, and patient advocacy across inpatient and outpatient settings.

Responsibilities include identifying patients for CM, developing care plans, coordinating with providers, and ensuring timely access to services. Requires nursing licensure, CM certification, and strong interdisciplinary teamwork.

Qualifications

• Minimum ASN, BSN preferred for Case Management; ACEN/CCNE accreditation is desirable.
• Three years of RN experience within last four years.
• Current, full RN license in any state.
• Certifications in CCM or other recognized case management specialties.
• BLS Healthcare Provider certification.

Responsibilities

• Provide nursing case management, care coordination and discharge/disposition planning for inpatient and outpatient settings.
• Identify patients for case management and collect medical, psychosocial and functional data.
• Develop patient-specific plans of care with goals and actions; coordinate with care team for approval.
• Guide patients and families through the healthcare system; coordinate referrals and services.
• Maintain documentation and data reporting per DoD/AF/facility guidelines.
